Do you take my insurance?

Perform Physical Therapy works with the following insurance plans: Aetna, CDPHP, Empire Plan, Excellus Blue Cross/Blue Shield, Humana, No Fault, Medicare, MVP, Wellcare, and Worker's Comp.

Becoming a participating provider with an insurance plan can take several months, so be sure to check back in as the list of insurance plans accepted continues to expand!

Question 2

Do I need a referral or prescription for physical therapy?

Accepted Answer

It depends on your insurance plan. In New York State, you can be seen by a Physical Therapist without a referral from a medical provider, for 30 days or 10 visits, whichever comes first. If you are still being treated beyond that, a referral would then become necessary.

However, certain insurance plans do require a referral to physical therapy. It's best for you to review your benefits and eligibility for your individual insurance plan before your first visit.

What should I wear to my appointments?

The more I can see, the easier it is for me to identify movement restrictions and visually assess for things like swelling and skin health. Loose style clothing is the easiest to work with, and shorts and t-shirt/tank top are preferred. Sneakers may not be necessary, especially on the first visit.

How long will my appointments be?

Accepted Answer

Your first appointment will be scheduled for 60 minutes. Follow up appointments are generally scheduled for 30 minutes.

How long do I have to come to physical therapy?

It depends! Your therapy plan will be individual to YOU, so there's no cookie cutter, pre-planned schedule. During your first appointment, we'll talk about what your goals are and what your availability is, and we will decide on the best plan that meets your needs.

Question 6

What if I can't make my appointment?

Accepted Answer

Life happens! It's totally understandable that you may need to cancel an appointment with me. The sooner you can notify me you won't be able to make it, the better. Cancellations with less than 24 hours' notice (or a no-show/no-call) may be subject to a cancellation fee of $40.
