What if my insurance plan is not listed?
If you do not see your specific insurance plan listed under the in-network section above, Perform Physical Therapy is considered an out-of-network (or "non-participating") provider.
Your plan may have out-of-network coverage for physical therapy. You will need to verify benefits with your specific plan.
At your request, a superbill can be provided for you to submit to your insurance plan for out-of-network coverage. However, payment to Perform PT is due in the full amount at the time of service.
You can pay your balance by cash, check, or credit card.

Can I pay privately and not use my insurance?
Absolutely!*
Here at Perform Physical Therapy, I believe the patient should be the one making the decisions about their care, NOT their insurance plan. Therefore, I offer patients the option to pay me privately for services and avoid their insurance plan all together.
Benefits of private pay:

No limit on number of therapy visits

No limit on covered/allowed treatments as part of your therapy program

Start/stop therapy when it makes sense for your health and wellbeing, rather than waiting for authorizations or stopping therapy because you've used up all your authorized visits

It's usually cheaper than paying towards insurance deductibles

It can actually cost you less than using your insurance plan benefits
